The pigment can be fine while its impurities aren't
This is the part that surprises makers who assume "the ingredient is on my supplier's approved list" is the end of the compliance question. Mineral pigments, because they come from geological sources, can carry trace levels of naturally co-occurring substances, certain heavy metals being the classic example, depending on the mine, the processing method, and the specific batch.

A COA for a mineral pigment batch should report the actual measured levels of relevant trace contaminants for that specific batch, not just confirm the pigment's primary identity and particle size.
If your supplier's documentation for a mica or iron oxide doesn't include impurity testing results, that's worth asking for directly before you use the batch in a product headed toward notification. They're surface-treated or coated, with silica, with a fatty acid coating, with a dispersant, to improve how they behave in a formula. Each of those treatments is itself an ingredient that needs its own INCI identification. A "treated mica" isn't automatically covered by checking plain mica against the Hotlist. Ask your supplier for the full composition of the treated pigment, not just the base mineral name, and map every component individually.

- Check both the base pigment and, where relevant, any documented impurity threshold against the current Hotlist before finalizing your formula for filing
- Keep the COA on file alongside your other supplier documentation for that product, since this is exactly the kind of detail a regulator could ask you to substantiate

None of this means mineral pigments are a compliance minefield to avoid. They're common, permitted, and well understood ingredients.
